Optavia is now Trilivy: what changed
Medifast, a Baltimore company that has sold meal replacements for decades, announced on July 17, 2026 that OPTAVIA would become Trilivy, a “metabolic health system” in three phases. The optavia.com address now opens the Trilivy site. The company said reformulated Reset Fuelings would debut in August 2026, and its site says they include a proprietary “MetaVantage” formula. The structure most people know (Fuelings, a Lean & Green meal and a coach) stays the same. Because many people still search for Optavia, this page uses both names.
Trilivy is a food and coaching program. It does not prescribe medication. Its site does talk about people who take GLP-1 medications and about using its plans while transitioning off them; if you are on a GLP-1, plan any diet change with the prescriber, and see our guides to eating on a GLP-1 and keeping weight off after a GLP-1.
How the plan works: Fuelings and Lean & Green
“Fuelings” are the company’s portioned meal-replacement products, delivered to your door (it also sells snacks and drink infusers). A “Lean & Green” meal is one you cook yourself. In the 2019 trial of the plan, a Lean & Green meal was described as 5 to 7 ounces of lean protein, three servings (about 1 1/2 cups) of non-starchy vegetables and up to two servings of healthy fat, providing 250 to 400 calories and more than 25 grams of protein. The 5 & 1 plan was designed to provide 800 to 1,000 calories a day with 80 to 120 grams of protein. The Fuelings have since been reformulated, so check the current nutrition labels.
The three Trilivy phases, as the company describes them

- RefineFor help keeping the weight off: "refine body composition and improve metabolic flexibility", in the company's words.
- Renew"Build your everyday wellness": a long-term phase the company links to "healthspan".
Eating 800 to 1,000 calories a day is far below what most adults usually eat, so the plan is not right for everyone. People who are pregnant or breastfeeding, teenagers, people with a history of an eating disorder, and people with diabetes (especially on insulin or sulfonylureas), kidney disease or heart conditions should only consider it with their clinician’s supervision. The company itself recommends contacting your healthcare provider before starting and throughout. The coach model
Support comes from independent Trilivy Coaches, a community and the company’s “Habits of Health” behavior system. The company says about 90% of its coaches started out as clients. Coaches can earn income from the program, and Trilivy publishes an income disclosure statement for coaches and states that it makes no guarantee of financial success. That business model (a network of independent sellers) means your coach is also, in part, a salesperson. Coaches are not described as licensed health professionals, so ask your coach what training they have, and keep medical questions for your doctor or a registered dietitian.
What the research shows
The company’s main study is a randomized trial published in Obesity Science and Practice in 2019. It enrolled 198 adults with overweight or obesity in the Chicago area (81% women, average BMI 34.2) and compared three groups for 16 weeks: the OPTAVIA 5 & 1 plan with telephone coaching, a self-guided Medifast 4 & 2 & 1 plan (1,100 to 1,300 calories a day), and a self-directed reduced-calorie diet based on federal guidelines. More than 92% finished the study. The trial was funded by Medifast, and several authors received fees from the company.

Body scans in the trial showed most of the weight lost was fat, and both meal-replacement groups lost about 0.24 kg of visceral (deep belly) fat. Lean mass fell less in the coached group (0.68 kg) than in the self-guided group (1.22 kg). The company’s “14% visceral fat reduction”, “98% of lean mass retained” and “10x more weight loss with a coach” claims all come from this one 16-week study. Keep in mind that the comparison group lost almost nothing, which makes the multiples look large, and that 16 weeks says little about the long term. The company also states that average weight loss on the Reset 5 & 1 plan is 12 pounds, with clients in the weight-loss phase for 12 weeks on average.
Independent evidence is limited. A 2015 systematic review in the Annals of Internal Medicine grouped Medifast with other very-low-calorie programs (Health Management Resources and OPTIFAST) and found they produced at least 4.0% more short-term weight loss than counseling, but the advantage shrank beyond six months when that was reported. In short: meal-replacement plans work well to start, and keeping the weight off depends on what comes after.

The Trilivy meal plans page lists a “starting at” price for each plan, and a 20% lower price with Premier+ autoship (automatic repeat orders). The page does not say how many days of food each order covers, so ask your coach before ordering, and remember the Lean & Green groceries are extra.

The company’s blog says Trilivy may be eligible for HSA or FSA spending and tells readers to check with their plan provider. The IRS generally allows weight-loss program fees for a disease diagnosed by a physician but not the cost of diet food that replaces normal meals, so ask your plan administrator before counting on it. Who it may suit
A Fuelings plan may suit adults who want fast early results, find decisions about food tiring, are comfortable eating mostly packaged products for a while, and value a personal coach. It may suit less well people who enjoy cooking, eat with a family, have a tight food budget, or want a plan with strong long-term independent evidence. If you like the structure of meal replacements but want a less restrictive calorie level, compare Nutrisystem and Jenny Craig. If you want flexibility and group support, compare WeightWatchers. A high-protein diet built from regular food is another option to discuss with a dietitian.

Every meal-replacement plan ends with a return to ordinary meals, and that is where weight regain usually starts. Ask your coach and your clinician how the Refine phase adds calories back, and how you will keep protein and activity up.
