What affects what you pay

  • Strength. The label describes starting at 0.25 mg weekly for four weeks, then 0.5 mg, with further increases to 1 mg and 2 mg at four-week intervals if needed for blood-sugar control. Self-pay prices are the same for 0.25 to 1 mg and higher for 2 mg.
  • Pen or tablet. Ozempic tablets are a separate product with different strengths; whether a tablet fits is a prescriber decision.
  • Insurance or self-pay. NovoCare says self-pay prescriptions are processed outside insurance, do not count toward deductibles and cannot be applied to out-of-pocket maximums.
  • Why it is prescribed. Plans usually cover Ozempic for type 2 diabetes; prescriptions for weight loss alone are off label and are often not covered.
  • Where it is filled. NovoCare offers free home delivery or pickup at a local pharmacy using its discount; retail cash prices differ.

Pens or tablets: the price difference

Ozempic now comes as a weekly pen and as a daily tablet (1.5, 4 and 9 mg), both approved for adults with type 2 diabetes. At NovoCare Pharmacy, tablets were $149 to $299 a month depending on the dose, compared with $349 to $499 for pens after the introductory offer, while both have the same $1,027.51 list price. The tablets have their own instructions (taken on an empty stomach with a little water, then waiting before eating), and the label says they cannot be swapped milligram for milligram with Rybelsus, another oral semaglutide for diabetes. Whether a tablet or a pen suits you, and at what strength, is for your prescriber to decide; lower cost alone is not a reason to switch without advice.

The Ozempic savings offer (the “coupon”)

People with commercial insurance that covers GLP-1 medicines may pay as little as $25 for up to a three-month prescription, with a maximum savings of $100 a month. Government beneficiaries are excluded. NovoCare adds that people with government insurance may still use its self-pay prices if they pay outside their insurance. Eligibility rules apply, and Novo Nordisk can change or cancel the program at any time; read the terms at SavingsCardEligibility.com and NovoCarePharmacyTerms.com. Novo Nordisk also runs a patient assistance program through which eligible people may receive medicine at no cost; ask NovoCare (1-888-693-6742) about current eligibility.

Medicare and Ozempic

Medicare Part D plans cover GLP-1 medicines prescribed for type 2 diabetes, heart disease, sleep apnea or MASH under their normal cost sharing, according to KFF. In 2026, Part D deductibles can be no more than $615, and once your out-of-pocket spending on covered drugs reaches $2,100 you pay nothing more for covered drugs that year. The Extra Help program lowers costs further for people with limited income. Medicare is barred by law from covering drugs used for weight loss, and Ozempic is not part of the temporary GLP-1 Bridge, which is for weight management (it covers Wegovy, Zepbound KwikPen and Foundayo).

Medicaid and commercial plans

Medicaid programs must cover drugs approved for treating diabetes, and, since 2024, for cardiovascular disease, according to KFF. That makes Ozempic for type 2 diabetes different from GLP-1 medicines used for obesity, which Medicaid covers only in some states (13 state fee-for-service programs as of January 2026). KFF notes that when GLP-1s are covered, they are typically subject to utilization controls such as prior authorization. Commercial plans also vary: ask whether Ozempic is on your formulary, its tier, whether prior authorization or a trial of another medicine is required first, and what your copay or coinsurance would be. What a year of self-pay Ozempic can cost

As an illustration only, using NovoCare Pharmacy’s self-pay prices on October 6, 2026: a new patient who paid $199 for each of the first two monthly fills and then $349 a month for ten fills at 0.25 to 1 mg would spend about $3,888 in a year. At the 2 mg strength the monthly price is $499, or $5,988 for twelve fills. Your prescriber sets your dose; with insurance and the savings offer the cost may be far lower, and on Medicare, Part D’s $2,100 cap limits what you pay for covered drugs in 2026.

Prices changing in 2027

Two changes are scheduled. First, Novo Nordisk announced on February 24, 2026 that the list price of Ozempic, Wegovy and Rybelsus will be $675 a month from January 1, 2027, and said its self-pay prices are not affected. A lower list price matters most to people with high-deductible plans or coinsurance, whose costs are often based on list price. Second, under the Medicare Drug Price Negotiation Program, Medicare’s negotiated price for Ozempic, Rybelsus and Wegovy will be $274 for a 30-day supply in 2027, a 71% discount from the 2024 list price of $959.

Ozempic vs Wegovy cost

Even though both are semaglutide, they are priced separately. On October 6, 2026, Wegovy’s list price was $1,349.02 per package, and NovoCare self-pay was $349 a month for most Wegovy pen strengths, $399 for the Wegovy HD pen and $149 to $299 for Wegovy pills. Counterfeits and unapproved copies

Low prices online can be a warning sign. The FDA has found counterfeit Ozempic 1 mg pens in the U.S. supply chain several times (December 2023, April 2025 and December 2025) and advises buying only from state-licensed pharmacies and checking the packaging. The government’s TrumpRx listing for Ozempic shows the same self-pay prices as NovoCare Pharmacy, which is one way to cross-check a price. Compounded semaglutide is not Ozempic and is not FDA approved; the FDA announced on February 21, 2025 that the semaglutide injection shortage was resolved. See compounded GLP-1s.

Ways to lower the cost

  1. Ask your plan about formulary status, tiers and any prior authorization for type 2 diabetes.
  2. Use the savings offer if you have commercial coverage; compare with self-pay if you do not.
  3. On Medicare, check Extra Help, and ask about the Medicare Prescription Payment Plan, which spreads out-of-pocket costs across the year as monthly bills (it does not lower the total).
  4. Ask about a 90-day supply, which NovoCare Pharmacy offers.
  5. Use HSA or FSA funds where allowed; the IRS counts prescribed medicines as medical expenses.
  6. Never stop a diabetes medicine because of cost without talking to your prescriber; ask about other options, including other diabetes medicines.
