How a GLP-1 pill differs from a GLP-1 shot

  • Daily vs weekly. All four pills are taken every day; the weight-management shots (Wegovy, Zepbound) are weekly.
  • Food and water rules. Semaglutide tablets must be taken on an empty stomach with a small sip of water, then a 30-minute wait before food, drinks or other oral medicines. Foundayo can be taken with or without food.
  • Molecule type. Semaglutide is a peptide; the Wegovy tablet contains an absorption enhancer (salcaprozate sodium, SNAC) listed among its ingredients. Only about 1% to 2% of a semaglutide tablet is absorbed, according to the Wegovy label. Orforglipron is a much smaller chemical molecule (molecular weight 902 g/mol in its label) rather than a peptide.
  • No needles, but the same class effects. The side effects and warnings are largely the same digestive ones seen with GLP-1 injections.
  • Who can use them. The Wegovy injection is also approved for teens 12 and older with obesity; the Wegovy pill and Foundayo are labeled for adults only.

What the trials showed

Wegovy pill. In the 64-week trial in the label (Study 7, also published as OASIS 4), 307 adults with obesity, or overweight with a weight-related condition, took semaglutide 25 mg tablets or placebo after a 12-week step-up, plus diet and activity counseling. Average weight change was -13.6% with the tablet and -2.4% with placebo. 76.3% of people on the tablet lost at least 5% of their weight, 59.8% at least 10%, 47% at least 15% and 27.9% at least 20%.

Foundayo. The label reports two 72-week trials. In Trial 1 (3,127 adults without diabetes, published as ATTAIN-1), average weight change was -7.4% on 5.5 mg, -8.3% on 9 mg and -11.1% on 17.2 mg, versus -2.1% on placebo. In Trial 2 (adults with type 2 diabetes), it was -5.1%, -7.0% and -9.6%, versus -2.5%.

Rybelsus and Ozempic tablets. These were studied for blood sugar in adults with type 2 diabetes, not for weight. In the 26-week monotherapy trial in their shared label, average weight change was -2.3 kg on 7 mg and -3.7 kg on 14 mg, versus -1.4 kg on placebo. That is why they are not a substitute for a weight-management label.

How they are taken, in general terms

Each label starts low and steps up every 30 days to limit stomach side effects; only your prescriber sets your dose. The Wegovy pill comes in 1.5, 4, 9 and 25 mg tablets, with 25 mg as the label’s maintenance dose. Foundayo comes in 0.8, 2.5, 5.5, 9, 14.5 and 17.2 mg tablets, increased at least 30 days apart based on response and tolerability, up to 17.2 mg; the label says to swallow tablets whole and not take more than one a day. Rybelsus (3, 7 and 14 mg) and Ozempic tablets (1.5, 4 and 9 mg) are not interchangeable milligram for milligram.

The semaglutide tablet routine, as the labels describe it

  1. First thing in the morningTake the tablet on an empty stomach.
  2. Water only, a littleSwallow it whole with up to 4 ounces (about half a cup) of plain water; no other drinks.

Why so strict? The Wegovy label’s pharmacology section reports that semaglutide absorption was higher after a longer fasting period following the dose, and higher with 50 mL of water than with 240 mL. It also notes that blood levels with the 25 mg tablet are predicted to be comparable to the weekly 2.4 mg injection, but more variable from person to person.

Foundayo has no empty-stomach rule. Its label does flag interactions: with strong CYP3A4 inhibitors (some antifungals and antibiotics, for example) the maximum dose is 9 mg; strong CYP3A4 inducers should be avoided; and simvastatin should not exceed 20 mg a day. Because these pills slow stomach emptying, they may affect how other oral medicines are absorbed.

Missed doses and switching, as the labels describe them

The labels treat missed pills differently. The Wegovy label says that if a tablet is missed, skip it and take the next one the following day. The Foundayo label says to take a missed dose as soon as possible without doubling up the next one, and that if 7 or more doses in a row are missed, the dose is restarted lower and stepped up again to limit stomach side effects. The Wegovy label also describes how adults can switch between the weekly injection and the daily tablet for weight or heart-risk reduction, and the Rybelsus label explains switching between Rybelsus and Ozempic tablets, which are not interchangeable milligram for milligram. Any switch, restart or skipped stretch is a conversation with your prescriber, not a do-it-yourself change.

Side effects

Digestive side effects are the most common. In Foundayo’s pooled trials, nausea affected 26-35% of people depending on the dose versus 10% on placebo, and vomiting 13-24% versus 4%. Other effects reported in at least 5% include constipation, diarrhea, indigestion, stomach pain, headache, bloating, fatigue, belching, reflux, gas and hair loss. In Rybelsus’s diabetes trials, nausea affected 20% of people on 14 mg versus 6% on placebo. For the Wegovy pill, the label says the types and frequency of common reactions were similar to the Wegovy injection; it also reports dysesthesia (unusual skin sensations) in 5% of tablet users and no placebo users.

Serious risks and boxed warnings

All four pills carry a boxed warning about thyroid C-cell tumors and are contraindicated with a personal or family history of medullary thyroid carcinoma or MEN 2. The Foundayo version notes that orforglipron is not pharmacologically active in rats or mice and did not produce tumors in rodents, while tumors were seen with GLP-1 products that are active in rodents; the human relevance has not been determined. Label warnings for the class include acute pancreatitis, severe digestive reactions, kidney injury from fluid loss, low blood sugar with insulin or sulfonylureas, allergic reactions, diabetic eye complications in people with type 2 diabetes, gallbladder disease and pulmonary aspiration during anesthesia or deep sedation. Foundayo is not recommended in people with severe gastroparesis or severe liver impairment.

Pregnancy. The Wegovy and Foundayo labels say the medicine may cause fetal harm and should be stopped when pregnancy is recognized. Talk with your prescriber before starting if you could become pregnant or are breastfeeding.

Cost without insurance (checked on 2026-10-06)

NovoCare Pharmacy lists the Wegovy pill at $149 a month for 1.5 mg, $199 for 4 mg and $299 for 9 mg and 25 mg (one month = 30 tablets). Lilly lists Foundayo’s regular self-pay price at $149 (0.8 mg), $199 (2.5 mg), $299 (5.5 and 9 mg) and $349 (14.5 and 17.2 mg) for 30 days, with temporary prices of $149 for 2.5 mg and $199 for 5.5 mg through December 31, 2026, and $299 for 14.5 and 17.2 mg when refills are on time under its purchase-offer terms. NovoCare lists Ozempic tablets self-pay from $149 a month, varying by dose. With commercial coverage, savings cards can lower copays to as little as $25 a month, with limits; government beneficiaries are excluded.

Medicare. The CMS Medicare GLP-1 Bridge (July 1, 2026 to December 31, 2027) includes all formulations of Wegovy (injection and tablets) and Foundayo for eligible Part D enrollees using them for weight, with a $50 copay and prior authorization. Compounded and “oral semaglutide drops”

Some websites sell compounded semaglutide as drops, dissolving tablets or capsules. The FDA warns that compounded and other unapproved GLP-1 products are not reviewed for safety, effectiveness or quality, that some use salt forms of semaglutide that differ from the approved ingredient, and that it knows of fraudulent products with false labels.
