How heavy should your vest be?
UCLA Health’s general advice is to start with a vest of about 5% of your body weight, so no more than 7.5 lb if you weigh 150 lb, and to build distance, weight and intensity gradually. If you are new to exercise, it suggests starting with unweighted walking, then, after several weeks, trying a weighted walk less than half as long as your usual one.

What the research says about weighted vests
Energy cost. A vest raises the effort of a walk, but a light one adds less than you might think on flat ground. In a lab study of 20 adults, a 14 lb (women) or 20 lb (men) vest raised the physiological demand of walking on a 10% incline but not on flat ground or a 5% incline. Hills, a brisker pace or a longer walk often add as much effort as the vest itself. Our calories burned calculator estimates walking at different speeds.
Bones and muscle during weight loss. Weight loss in older adults is linked with some loss of bone and muscle, and a vest has been studied as a way to “replace” the lost weight. In the INVEST in Bone Health trial, 150 adults with obesity (average age 66) lost about 9% to 11% of their weight over 12 months. One group wore a weighted vest about 7 hours a day, replacing most of the lost weight. Hip bone density fell in every group, and the vest did not prevent that loss compared with weight loss alone. A follow-up analysis found that the group doing supervised resistance training kept thigh muscle better than the vest group.

So a weighted vest is a reasonable way to make walking harder, but the evidence doesn’t support it as a substitute for strength training or as a way to protect bones during weight loss on its own.
